CHILDREN’S HEALTH SERVICES NAVIGATOR

Gold River Worksite and District

Under the direction of the Vancouver Island West Early Learning Committee and the supervision and evaluation of the District Principal of Student Learning and Staff Development, the Children’s Health Services Navigator assists families to navigate health services for children in School District 84.  

The Children’s Health Hub is located in the Village of Gold River on the West Coast of Vancouver Island, 90 km from Campbell River.  Nestled among snow-capped mountain peaks, surrounded by the Pacific Rain Forest and open to the waters of the historic Nootka Sound, Gold River is an ideal location for outdoor enthusiasts.  With its beautiful rivers, breathtaking scenery, clean air, excellent water, and affordable living, Gold River provides the foundation for healthy living.   

This appointment is effective immediately and has a flexible work schedule equal to 20 hours per week, when schools are in session.  After completion of a successful probationary period, the incumbent will be eligible for full benefits in accordance with the Collective Agreement between the Board of Education of School District 84 (Vancouver Island West) and the Canadian Union of Public Employees, Local 2769.

Duties

  • Identifies and maps available children’s health services.

  • Identifies gaps in children’s health services.

  • Navigates current and future health services for children.

  • Builds relationships with community organizations, services providers and families throughout School District 84.

  • Liaises between children’s health services providers and families to facilitate regular communication and services.

  • Coordinates and schedules services.

  • Gathers information, completes reports (including budgets) and makes presentations as required.

  • Travels to provide outreach to families and communities throughout School District 84 (Gold River, Tahsis, Zeballos, Kyuquot).

  • Attends community and regional meetings.

  • Possesses a strong understanding of child development and demonstrates a strong ability to work with children and families with diverse needs and challenges.

  • Establishes and maintains trust and rapport with children and families.

  • Works independently to engage families, communities and service providers. 

  • Maintains complete and current records.

  • Operates standard office equipment.

  • Performs all duties with a level of interpersonal skills appropriate to the position.

  • Maintains the confidentiality of sensitive information seen or heard.

  • Performs other job-related duties as assigned.

    Qualifications

  • Grade 12 education or the equivalent including fluent proficiency in written and spoken English;

  • Two-year post-secondary diploma in Human Services, Social Services or equivalent; 

  • Over one and up to two years  of experience liaising with local and regional community services including health services, children’s services and others;

  • Proven ability to work independently and with others to ensure success;

  • Demonstrated ability to produce reports, budgets and presentations using standard office software application;

  • Possesses local knowledge of family systems and culturally appropriate practices;

  • Physical capability to perform the job duties; and,

  • Valid BC driver’s license and clear driver’s abstract.

    The temporary rate of pay is $26.00 per hour, to be reviewed by the Joint Job Evaluation Maintenance Committee

    after six months.  Employment is subject to an approved Criminal Record Check.
